Please complete ALL requests below:

1. COMPLETE Online Registration Form

Please make sure that contact information (cell # and email addresses)
provided on the registration form are accurate and are frequently monitored

2. PRINT and SIGN Forms
REQUIRED: Please bring SIGNED forms to the Welcome Meeting on September 7th
(Parent and Player Signature required)

3. Pay for Registration Donation
